In efforts to keep people within our community healthy, please be aware that by booking an appointment you confirm that, in the last 14 days, neither you nor anyone else in your household has had any of symptoms of Coronavirus as outlined by the Centers for Disease Control (“CDC”) including:
– Fever
– Tiredness
– Dry cough
– Aches and pains
– Nasal congestion
– Runny nose
– Sore throat
– Diarrhea
– Loss of taste or smell
Additionally, you confirm that you:
– Have not been diagnosed with, or told by a medical professional to presume you have, Coronavirus in the last 30 days or, if you have been diagnosed with or told to presume you have Coronavirus, you have been symptom free without the use of fever-reducing medication for at least 14 days.
– Have no one living in the same household who has been diagnosed with, or told by a medical professional to presume you have, Coronavirus in the last 30 days or, if a household member has been diagnosed with or told to presume he or she has Coronavirus, he or she has been symptom free without the use of fever-reducing medication for at least 14 days.
– Have not traveled internationally within the past 14 days
– Have not been exposed to Coronavirus within the past 14 days
– Have no one living in the same household who has been exposed to Coronavirus within the past 14 days.
By requesting a massage therapy appointment, I hereby certify that all of the above statements are true and accurate to the best of my knowledge. I understand that nothing in this form is intended to be relied on by me to make any decisions about whether I have Coronavirus and I will not rely on this document or the decision of the massage therapist to honor my massage appointment in determining whether I have Coronavirus. I also understand that my massage therapist has affirmed the same and has agreed to adhere to COVID-19 sanitation and safety protocols. I further understand that, even in light of these procedures, there is still a risk that I can contract COVID-19 as a result of my interactions with the massage therapist. I knowingly and voluntarily assume this risk and hereby release, forever discharge, and hold harmless the massage therapist from any and all liability, claims, or demands, of whatever nature, arising from or related to the unintentional exposure or harm due to COVID-19 for any reason whatsoever.
